Karlovy Vary is worth seeing for its architecture alone. Not only the spa district with its spa architecture, but also the inner city bear witness to the architecture at the turn of the 19th and 20th centuries. The spa district is only populated with a few cafés out of season, while the existing facilities are often overcrowded. Nevertheless, a walk to either side of the river is unique. Don't forget to visit the museum. There you will learn a lot about the history and development of the place. Another highlight is the Becherovka Museum. You don't necessarily need a car to visit the city. The spa hotels often offer cheap, sometimes even free pick-up from home. You save money on gas and parking fees. The city center and the spa district are pedestrian-friendly.
